How to Become a McBurney Client

1. Contact McBurney.

Call our office to provide McBurney with your student ID number and other contact information.

If you haven't applied to UW-Madison yet, you'll be asked for your security number so we can link you to Admissions Office data and begin providing transition services when you are accepted to UW-Madison.



2. Complete Accommodation Request Form and return to McBurney.

• Download the Accommodation Request Form in Microsoft Word Format.



3. Provide disability documentation:

• Read Documentation Guidelines. Forward these guidelines to the clinician providing your documentation.



• Have clinician complete Disability Assessment Form (link opens in Microsoft Word Format) and forward it to McBurney.
  
  NOTE: Students with learning disabilities do not need to provide this form. Please see Learning Disability Documentation Guidelines for Students at the University of Wisconsin-Madison for LD documentation requirements.



• Send to McBurney any records of accommodations/services provided to you at any previous academic settings (e.g., IEP, Section 504 plan, college service plan, etc.) and any record of accommodations provided for any national standardized exams (e.g., ACT, SAT, GRE, LSAT, etc.).


NOTE: If you suspect you may have a disability but have never been diagnosed, see the Learning Assessment and Resources page.



4. Schedule a first intake appointment.

• Current UW Madison Students and Students Admitted to UW-Madison:


Contact our front desk to schedule an appointment with the appropriate Accommodations Specialist. Make sure your disability documentation arrives at least a week prior to this appointment.


• Students Not Yet Admitted:


Until you are currently enrolled or admitted to UW-Madison, you do not need to schedule an intake appointment. Please refer to our Prospective Student page for more information.



5. Eligibility for services is determined.

See additional information regarding determining eligibility for services.

Admitted Freshman and Transfer Students: If you need an eligibility determination before you pay your enrollment deposit, please send a written request to McBurney.



6. Accommodation/service plan called a VISA is developed.

After you complete your intake appointment and are eligible for services you may view your VISA (Verified Individualized Services and Accommodations). This is your recommended accommodations plan.

7. Attend service training/orientation for services that require it.

8. Request services.

You will receive instructions for requesting services at the service training sessions or from your accommodations specialist.



9. Follow-up, as needed.
